The Blue Cross Blue Shield of GA Individual/Family Dental Discount Plans require that premiums be paid by automatic bank draft. For the Automatic Bank Draft Authorization Form, click here. This form must be submitted with the application.
The premiums for the SmileNet Dental Discount Plans can be paid by credit card or check.
